To enroll at IAS, students must complete a pre-enrollment form. You can fill out the easy online form below or you can download a PDF version and drop it off at our main office.

 New Student Checklist

Parents of new students must bring the following documents to the enrollment appointment:

  • Identification (Photo ID, such as a driver’s license, state ID of parent or legal guardian)

  • Birth certificate – Student’s original birth certificate with a raised seal of authenticity, as required by law

  • Immunization records – Please supply the most recent record

  • Proof of Residency documents
